The Harry Meyering Center, Inc. offers
Support Planner Services (formally known as “Flexible Case
Management”). The role of the Support Planner is to develop a
plan and the corresponding budget with the client that allows
that person to control the services he/she needs to remain in
the their home of choice. The plan details the personal
supports, hands-on assistance and respite care, therapy,
training, supplies/equipment, modifications, assistive
technology, transportation and a health and safety plan
necessary to live successfully in their home.
The Support Planner works with the
consumer, their family, and the county of financial
responsibility to choose and design the services and supports
that fit their need, when to receive those services and who will
deliver those services.
Support Planner services can be purchased
privately or using Consumer Directed Community Supports (CDCS)
under any Minnesota waiver program including MR/RC, Elderly, and
CADI, CAC and TBI waivers.
