Community First Services and Supports (CFSS)
What is CFSS?
Community First Services and Supports (CFSS) is the program that is replacing Personal Care Assistance (PCA) programs, including traditional PCA and PCA Choice and the Consumer Support Grant (CSG) programs. CFSS is funded through Medical Assistance (MA), waiver, and Alternative Care programs.
The Minnesota Department of Human Services (DHS) has designed the CFSS program to offer participants more choice, more control, and flexibility with the services and supports needed to live and work in the community.
The CFSS program provides assistance with:
- Activities of daily living (ADLs), such as eating, bathing, grooming, dressing and transferring
- Instrumental Activities of Daily Living (IADLs) such as companionship, shopping, cooking, household management, medication management, managing finances
- Health-related tasks
- Back up to ensure continuity of services
- Services and supports that increase a person’s independence
- Items that provide assistance in meeting a need without requiring human assistance
- Skill acquisition
Why choose HMC for your consultation services?
The HMC facilitator will provide you with education, information, and assistance as needed to make an informed choice on the service delivery model that best meets your needs, assist in developing a service delivery plan, and provide information about monitoring and evaluating services.
Under CFSS you will have the choice of two service delivery models – the Agency-Provider Model and the Budget Model. The facilitator will provide information about both models to allow you to choose the model that best fits your needs.
The facilitator will assist you to develop goals and outcomes that will meet your needs.
HMC will be available to provide support and guidance throughout the year and assist with quality improvement as needed.
You will design person-centered services to meet your needs that fit your lifestyle.
You may choose to have a representative who is authorized to act on your behalf in making the decision for CFSS.
